File a Complaint
1. Eligibility to File a Claim
Claims may be filed by patients, family members, healthcare professionals, or any individual with direct knowledge of the incident.
Claims must relate to actions or omissions by Care We Trust or its employees that are in violation of applicable laws or regulations.
2. Submission Process
Claims must be submitted in writing and can be sent via mail downloading the form linked below.
A completed complaint form must accompany all submissions. Forms can be obtained from Care We Trust website or by request.
3. Required Information
Claimants must provide the following information:
Claimant's name and contact information
Details of the incident (date, time, location)
Names of individuals involved
Description of the alleged violation
Any supporting documentation (e.g., medical records, correspondence)
4. Confidentiality
All claims and investigations will be treated with confidentiality to the extent possible. However, the registry may be required to disclose information in accordance with applicable laws.
5. Investigation Process
Upon receiving a claim, the we will conduct an initial review and may initiate an investigation.
Claimants may be contacted for additional information or clarification during this process.
6. Outcome Notification
Claimants will be notified of the outcome of the investigation within a specified timeframe, as required by regulation.
Outcomes may include findings of no violation, recommendations for corrective action, or further disciplinary measures.
7. Appeals
Claimants have the right to appeal the outcome of the investigation if they believe it to be unjust. Details regarding the appeal process will be provided with the outcome notification.
8. Compliance with Regulations
All claimants must comply with the Aspen State Regulation Set: G 3.06 and any other applicable laws during the claim process.
9. Amendments
Care We Trust reserves the right to amend these terms and conditions at any time, with notice provided to claimants as required by law.
